Chloes House

Chloe's House is a care agency that offers non-medical support to individuals with disabilities, mental health conditions, substance use disorders, and seniors. Our primary objective is to help individuals and families who need help finding housing and sustaining housing.  We collaborate closely with our clients to create customized plans that meet their needs and preferences.

 

Our top priority is meeting each client's unique needs so that they can maintain their independence at home and making sure we help keep them in their home. We take pride in providing personalized services to our clients.

 

Our Housing Consultation service helps clients develop a plan focused on housing. Housing Transition assists clients in finding a new home in the community, while Housing Sustaining provides support to help clients maintain their homes.

We take great pride in what we do and strive to meet the needs of our clients, allowing them to become or remain independent in their homes.
